These Terms incorporate (and you agree to) our:

• HIPAA Notice of Privacy Practices (where applicable to protected health information)
• Any additional policies posted on our site (including practice policies)

The Services are intended for individuals 18 years of age or older. By using the Services, you represent and warrant that you are at least 18 and have the legal capacity to enter into these Terms.

Do not use PureRx for medical emergencies. If you believe you are experiencing an emergency, call 911 immediately or go to the nearest emergency room.

PureRx provides access to a health platform that may include intake, lab coordination, and telehealth visits with licensed clinicians. Your clinician is the ultimate authority in determining whether you are a candidate for any therapy and what treatment, if any, is appropriate.

You understand and agree:

• A clinician may determine that telehealth is not appropriate for your needs.
• No prescription, therapy, or medication is guaranteed.
• Any treatment plan is individualized and may change based on your health information, lab results, and clinical judgment.

PureRx may coordinate diagnostic lab testing and, where clinically appropriate, coordinate prescriptions and medication delivery through third-party labs, pharmacies, and fulfillment partners.

You acknowledge:

• Labs and pharmacies may have their own terms and policies.
• Delivery timelines can vary, and PureRx is not responsible for delays outside our control.
• You are responsible for providing accurate information for shipping and contact.

You may be required to use a patient portal to access certain Services (e.g., scheduling, forms, results, messaging).

You agree to:

• Provide accurate, current, and complete information
• Maintain the confidentiality of your login credentials
• Notify us promptly of any suspected unauthorized use of your account

You are responsible for all activity that occurs under your account.

You may be asked to “text us” to begin the intake process or communicate about Services. By providing your phone number and using the Services, you consent to receive communications from PureRx (including via SMS/text, phone calls, or email) related to your care, scheduling, account, and service updates. Standard message and data rates may apply.

You can opt out of promotional texts at any time by replying STOP. Certain service-related messages may still be sent when permitted by law (e.g., essential scheduling or account notifications).

You agree to pay all fees associated with Services you purchase, including consults, memberships, labs (if not included), and any other offerings presented at checkout or during enrollment.

Order limitations: We reserve the right to reject or limit any order or purchase, including limiting quantities, for any reason. If we reject an order, we will attempt to notify you using the contact information you provided.

Payments: Payments may be processed through third-party payment processors. You authorize us (and our payment processors) to charge your selected payment method for authorized purchases.

If PureRx offers coaching or educational services, those services are provided for general educational and wellness purposes and do not replace medical care. You agree you will not:

• Upload, post, transmit, or distribute any content that infringes another party’s rights
• Use the Services for any unlawful purpose or in violation of applicable laws
• Send spam, unsolicited messages, or promotional materials to others through the Services
• Post or transmit content that is unlawful, threatening, harassing, abusive, defamatory, obscene, hateful, or otherwise objectionable
• Attempt to probe, scan, or test the vulnerability of any system or network
• Interfere with or disrupt the integrity or performance of the Services
• Attempt to gain unauthorized access to accounts, systems, or data

All content on the Site and within the Services—text, graphics, logos, design, videos, and other materials—is owned by or licensed to PureRx and protected by intellectual property laws. You may not copy, reproduce, distribute, modify, or create derivative works without our prior written consent, except as permitted by law.

If you submit content (including testimonials, reviews, comments, messages, or other materials), you represent that you have the right to do so. You grant PureRx a non-exclusive, worldwide, royalty-free license to use, display, reproduce, and distribute such content for operating and marketing the Services, unless prohibited by law.

We may remove content that violates these Terms or is otherwise objectionable, at our discretion.

Information provided on the Site is for informational purposes only and is not medical advice and not a substitute for professional evaluation, diagnosis, or treatment. Individual results vary and outcomes are not guaranteed.

You may not attempt to bypass security measures or tamper with the Site or Services. Unauthorized intrusion, interference, or misuse is strictly prohibited.

To the fullest extent permitted by law, the Services are provided on an “AS IS” and “AS AVAILABLE” basis, without warranties of any kind, express or implied, including merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that the Services will be uninterrupted, error-free, or completely secure.

To the fullest extent permitted by law, PureRx and its affiliates, officers, directors, employees, contractors, agents, and licensors will not be liable for any indirect, incidental, consequential, special, or punitive damages arising from or related to your use of (or inability to use) the Services.
